If you plan to use a smartphone or tablet, you may be asked to first download the Simple Practice Telehealth App (Apple, Android).
Parking: Garage parking is available for $5/hr (cash only). Metered street parking is often available on Lankershim Blvd.
Upon Arrival: Please take the elevator to the 5th floor for Suite 545. Please have a seat once inside, we’ll greet you at the time of your appointment.
